STARBASE MN Security Form

Note: To complete this security form, you will need the driver license number and date of birth for any adult that will be attending STARBASE with your class. Each classroom teacher needs to submit a security form.

Due to security requirements at the Minnesota Air National Guard Base, all adults entering the base must show a photo ID upon entry. In addition, several days prior to coming to STARBASE Minnesota, any adult who will be entering the base must provide the information below to STARBASE so it can be routed through security. We ask that all adults attending with the class, including parents, ride the bus with the students. Please fill out and submit the information below five days before coming onto the base. Thank you.

STARBASE does not retain participants’ driver’s license number information on district employees after their participation at STARBASE for security purposes. This information is required each time a person participates.
